Tryout Forms
All paperwork will be available at Tryouts.  If you are not able to print this information ahead of time, you will be able to fill out all necessary information on site.

If you would like to download these forms prior to tryouts you may do so and bring them with you. Please put forms with numbers in the order designated below, staple, and bring with you to tryouts. The forms without numbers are for your benefit only.  You do not need to turn these forms in at tryouts.

Please still try to come early to turn in your paperwork, get your number, and register at tryouts.

Everything you need to know to be ready for tryouts
     
1. At least 1-2 weeks prior to Tryouts.  Go to the North Texas Regions website and register for your 2008 USA Volleyball membership.  Junior athletes MUST be registered and recorded as paid prior to attending any tryout.

  a. Follow the instructions and fill out an online registration.
  b. During the registration, choose “Undecided” when asked to “choose a club”. 
  c. After you have accepted an offer from a club, then you can go back and select a club.
  d. Once the region office has received fees, they will update the record to reflect payment. Parents will receive an automated email stating their child is a member. Parents should print that email and take to tryouts or they may log in and print a membership card.
     
2. Fill out the Skyline Tryout Forms

  b. USAV Medical History and Release Form
  c. Skyline Assumption of Risk
     
3. Bring $75 to cover the cost of tryouts (one-time fee for all tryouts)
     
4. Come early to turn in paperwork, get your number, and register.
     
     
If you are still in High School playoffs...
  It is a North Texas Region policy that you are not allowed to tryout until your High School season is finished. However, we would like for you to still come to the tryout and fill out the necessary paperwork. We also would like the athletes to sit and watch during the tryouts. We also ask that you notify us in advance, so we can try to come watch one of your matches.  If you have a video, please try and get it to us prior to tryouts.
   
If you are still in your Junior High season...
  The above policy is a courtesy to the High School coaches to limit the chances of a high school athlete getting hurt and it effecting their schools chances in the State playoffs.  You are allowed to tryout during your Middle School Season.  If our tryouts directly conflict with one of your 7th or 8th grade team matches, please let us know in advance so we can try to come watch one of your previous matches.
