For the flight - all uniforms, knee pads, socks, shoes, spandex need to be in your Skyline bag for carry on. This will be the only bag you can carry on, so make sure you put everything you want on the plane in this bag. PLEASE DO NOT FILL IT UP AS FULL AS IT WILL GO.

 All players need to wear their skyline warm-ups to help speed up the check-in process.

Everyone must have a CURRENT Drivers license for check - in.

If your parent's are not traveling with you on this trip, to be released to ride with anyone other than your coach or designated chaperone, you will need to have this form signed by your parents and turned into your coach.
